| Submit Claims: |
| IHC Health Solutions has partnerships with more than 60 PPOs. To submit electronic claims to IHC Health Solutions
you will need to know the PPO (if any) indicated on the front of the patient’s
ID card. |

| PHCS, First Health, Beech Street, Interplan, Evolutions, InterWest, Ohio Health Choice, Multiplan or Nevada Preferred:
Submit your electronic claims to Emdeon with our payor ID of 86304, or to ENS or Availity
with our payor ID of IAC01. Submit paper claims to the address shown on the
card: IHC Health Solutions, PO Box 21456, Eagan, MN 55121-0456. Should you require further
assistance, please call us at 815-316-6710. |

| For all other PPOs: If you are a participating provider in the PPO
indicated on the card, follow the instructions you have received from that PPO
to submit electronic claims to them. |

| If there is no PPO Network logo on the front of the card OR if you are not a
participating provider: Submit your electronic claims to Emdeon
with our payor ID of 86304, or to ENS or Availity with our payor ID of IAC01.
